29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The employer did not establish an energy control program consisting of lockout tagout procedures, employee training, periodic inspections, and group lockout tagout procedures when employees were engaged in servicing and maintenance tasks on equipment such as the routers, up-cut saws, table saws, sander, planer, and dust collectors among others.

29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.   The employer did not develop, document, and utilize energy control procedures when employees performed servicing and maintenance work on equipment such as the routers, up-cut saws, table saws, belt sander, planer, and dust collectors among others.  Note: Exception: The employer need not document the required procedure for a particular machine or equipment, when all of the following elements exist: (1) The machine or equipment has no potential for stored or residual energy or reaccumulation of stored energy after shut down which could endanger employees; (2) the machine or equipment has a single energy source which can be readily identified and isolated; (3) the isolation and locking out of that energy source will completely deenergize and deactivate the machine or equipment; (4) the machine or equipment is isolated from that energy source and locked out during servicing or maintenance; (5) a single lockout device will achieve a locker-out condition; (6) the lockout device is under the exclusive control of the authorized employee performing the servicing or maintenance; (7) the servicing or maintenance does not create hazards for other employees; and (8) the employer, in utilizing this exception, has had no accidents involving the unexpected activation or reenergization of the machine or equipment during servicing or maintenance.

29 CFR 1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  The employer did not provide employees training in an energy control program when they engaged in servicing and maintenance work on equipment such as the routers, up-cut saws, table saws, belt sander, planer, and dust collectors among others.  The employer shall train authorized employees, affected employees, and employees whose work operation are or may be in the area where energy control procedures may be utilized.

29 CFR 1910.178(l)(1)(i): The employer shall ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l).    The employer did not train and evaluate forklift operators prior to allowing them to load and unload wood and cabinets in the shop and loading dock.  Training shall consist of a combination of formal instruction, practical training, and an evaluation of the operator's performance.

29 CFR 1910.213(c)(1): Each circular hand-fed ripsaw shall be guarded by a hood which shall completely enclose that portion of the saw above the table and that portion of the saw above the material being cut. The hood and mounting shall be arranged so that the hood will automatically adjust itself to the thickness of and remain in contact with the material being cut but it shall not offer any considerable resistance to insertion of material to saw or to passage of the material being sawed. The hood shall be made of adequate strength to resist blows and strains incidental to reasonable operation, adjusting, and handling, and shall be so designed as to protect the operator from flying splinters and broken saw teeth. It shall be made of material that is soft enough so that it will be unlikely to cause tooth breakage. The hood shall be so mounted as to insure that its operation will be positive, reliable, and in true alignment with the saw; and the mounting shall be adequate in strength to resist any reasonable side thrust or other force tending to throw it out of line.    (a)  On or about February 21, 2018, a Delta X Unisaw table saw located in the shop was not equipped with a hood that enclosed the portion of the saw above the table.    (b)  On or about February 21, 2018, a Powermatic table saw located in the door area was not equipped with a hood that enclosed the portion of the saw above the table.

29 CFR 1910.213(c)(2): Each hand-fed circular ripsaw shall be furnished with a spreader to prevent material from squeezing the saw or being thrown back on the operator. The spreader shall be made of hard tempered steel, or its equivalent, and shall be thinner than the saw kerf. It shall be of sufficient width to provide adequate stiffness or rigidity to resist any reasonable side thrust or blow tending to bend or throw it out of position. The spreader shall be attached so that it will remain in true alignment with the saw even when either the saw or table is tilted. The provision of a spreader in connection with grooving, dadoing, or rabbeting is not required. On the completion of such operations, the spreader shall be immediately replaced.  (a)  On or about February 21, 2018, a Delta X Unisaw table saw located in the shop was not equipped with a spreader to prevent material from squeezing the saw or being kicked back.    (b)  On or about February 21, 2018, a Powermatic table saw located in the door area was not equipped with a spreader to prevent material from squeezing the saw or being kicked back.

29 CFR 1910.213(g)(2): Each swing cutoff saw shall be provided with an effective device to return the saw automatically to the back of the table when released at any point of its travel. Such a device shall not depend for its proper functioning upon any rope, cord, or spring. If there is a counterweight, the bolts supporting the bar and counterweight shall be provided with cotter pins; and the counterweight shall be prevented from dropping by either a bolt passing through both the bar and counterweight, or a bolt put through the extreme end of the bar, or, where the counterweight does not encircle the bar, a safety chain attached to it.  On or about February 21, 2018, a Rigid 12" sliding miter saw located in the molding and specialty area did not automatically return as required.

29 CFR 1910.305(b)(1)(i): Conductors entering cutout boxes, cabinets, or fittings shall be protected from abrasion, and openings through which conductors enter shall be effectively closed.    On or about February 21, 2018, conductors entering the control cabinet and electrical motor for the Powermatic table saw located in the door area were not protected from abrasion as the insulation was pulled back and the opening was not effectively closed.
