29 CFR 1910.67(c)(2)(v): Personal fall arrest or travel restraint systems that met the requirements in 29 CFR part 1910, subpart I, were not worn and attached to the boom or basket when working from an aerial lift: a) Job site, 1530 Chartiers Avenue, Pittsburgh, PA: On or about February 21, 2018, during troubleshooting activities, the employer did not ensure that an employee used appropriate fall protection systems/devices while working in the bucket of an aerial lift at a height of up to approximately 13 feet.
