The cord was being used in lieu of permanent wiring exposing employees in the area to an inhalation hazard in the event of an electrical fire.    (b) On or about February 22, 2018 a flexible cord with a power strip end attached to it was found in the mixing area powering a fan which was hanging from the buildings structural members by chains. The cord was being used in lieu of permanent wiring exposing employees in the area to an inhalation hazard in the event of an electrical fire.    (c) On or about February 22, 2018 a flexible cord was found in the mold shop powering a fluorescent light fixture which was attached to a non-operating permanently mounted fluorescent light fixture by the use of electrical wiring causing it to hang below it. The cord was being used in lieu of permanent wiring exposing employees in the area to an inhalation hazard in the event of an electrical fire.

The cord was attached to building surfaces by tying it and using plastic zip ties exposing employees in the area to an inhalation hazard in the event of an electrical fire.  (b) On or about February 22, 2018 a flexible cord with a power strip end attached to it was found in the mixing area powering a fan which was hanging from the buildings structural members by chains. The cord was attached to a beam and pipe with plastic zip ties and tape exposing employees in the area to an inhalation hazard in the event of an electrical fire.  (c) On or about February 22, 2018 a flexible cord was found in the mold shop powering a fluorescent light fixture which was attached to a non-operating, permanently mounted fluorescent light fixture by the use of electrical wiring causing it to hang below it. The cord was tied back to the wall of the structure with a plastic zip tie exposing employees in the area to an inhalation hazard in the event of an electrical fire.
