29 CFR 1910.303(h)(2)(v)(E): Doors and covers of enclosures used solely as pull boxes, splice boxes, or junction boxes were not locked, bolted, or screwed on to prevent unauthorized personnel access. Employees performing administrative duties in the admin offices were exposed to electrical shock hazards in that the top of panel D which was being used solely as a junction box was not locked or otherwise secured to prevent unauthorized access.
