29 CFR 1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket while working from an aerial lift: a) Shoppes at Belmont, Lancaster PA - An employee was observed working from the basket of a GMC C5500 bucket truck installing building sign letters and was not wearing a body belt (or harness) with lanyard attached to the boom or basket and was exposed to an approximate 15 foot fall hazard to the ground below, on or about February 20, 2018. Apex Maintenance Group, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.67(c)(2)(v), which was contained in OSHA inspection number 966096, citation number 1, item number 1, and was affirmed as a final order on May 6, 2014, with respect to a workplace located at 5020 Jonestown Road, Harrisburg, Pennsylvania 17112.
