29 CFR 1910.28(b)(1)(i): (1) Unprotected sides and edges.  (i) Except as provided elsewhere in this section, the employer must ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the following: Guardrail systems; safety net systems, or personal fall protection systems, such as personal fall arrest, travel restraint, or positioning systems.    On or about February 23, 2018, employees were exposed to fall hazards of approximately 20 feet when opening and closing the crane opening on top of the West blast booth.

29 CFR 1910.30(a)(1): (a) Fall hazards. (1) Before any employee is exposed to a fall hazard, the employer must provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart.  Employers must ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.    On or about February 23, 2018, employees were exposed to fall hazards over four feet without adequate training regarding the nature of fall hazards in the work area and how to recognize them, the procedures to be followed to minimize those hazards, and correct procedures for using fall protection systems.

29 CFR 1910.140(c)(13)(i): (i) Anchorages, except window cleaners' belt anchors covered by paragraph (e) of this section, must be capable of supporting at least 5,000 pounds (22.2 kN) for each employee attached; or Designed, installed, and used, under the supervision of a qualified person, as part of a complete personal fall protection system that maintains a safety factor of at least two.      (a)  On or about February 23, 2018, anchor points used for loading and unloading semi-trailers utilized turn buckles rated to 2250lbs.      (b)  On or about February 23, 2018, anchor points used in the west blast booth to access the top of tanks utilized turn buckles rated to 3500 lbs.

29 CFR 1910.23(b)(9): (9) Ladders are inspected before initial use in each work shift, and more frequently as necessary, to identify any visible defects that could cause employee injury;  On or about February 23, 2018, a 6 foot fiberglass ladder located in the load bay area was coated with overspray inhibiting employees ability to visibly inspect it for defects prior to use.
