29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition:    Milling Area:  On or about February 23, 2018, and at times prior thereto, employees were exposed to fire hazards, as there were accumulations of wood dust greater than 1/8 inch in depth on vertical and horizontal ledges, saw motors, electrical equipment, and on the ground throughout the establishment.

29 CFR 1910.28(b)(11)(i): Each employee exposed to an unprotected side or edge of a stairway landing that was 4 feet (1.2 m) or more above a lower level was not protected by a guardrail or stair rail system:    a)  Hammer mill area: On or about February 23, 2018, and at  times prior thereto, employees were exposed to a fall hazard from a stair rail system leading up to the second level that was not equipped with a connecting mid-rail to the stairway structure.    b)  Drum storage area: On or about February 23, 2018, and at times prior thereto, employees were exposed to a fall hazard from a stair rail system leading up to the second level that was not equipped with a mid-rail to the stairway structure.     c)  Hog mill area: On or about February 23, 2018, and at times prior thereto, employees were exposed to a fall hazard from a stair rail system leading up to the second level that was not equipped with a mid-rail to the stairway structure.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:    a) Work shop area:  On or about February 23, 2018, and at times prior thereto, employees were exposed to electrical hazards when having to enter the shop to access electrical panel boxes to turn lights on and off in the yard.      b) Hammer mill area:  On or about February 23, 2018, and at times prior thereto, employees were exposed to electrical hazards while working near a Siemens Induction Motor which had exposed electrical wires from the broken conduit.    c) Hog mill area:  On or about February 23, 2018, and at times prior thereto, employees were exposed to electrical hazards when energizing the hopper motor which had exposed electrical wires from the broken conduit.
