J.A.C.K. Wood Products, LLC, various areas throughout the facility, on or about February 23rd, 2018 and times prior thereto:  Employees worked with and around hazardous substances including, but not limited to wood dust, diesel fuel during work operations at the facility.  The employer did not develop and maintain a written Hazard Communication Program.

Wood Products, LLC, various areas throughout the facility, on or about February 23rd, 2018 and times prior thereto:  Employees worked with and around hazardous substances including, but not limited to wood dust, diesel fuel during work operations at the facility.  The employer did not provide effective Hazard Communication training for those employees working with and around hazardous substances.

Wood Products, LLC, various areas throughout the facility, on or about February 23rd, 2018 and times prior thereto:  Employees voluntarily used disposable filtering facepiece respirators (dust masks) during cleaning operations of wood dust onsite.  Employees that used the dust masks were not provided with the information in Appendix D to this section.
