29 CFR 1910.253(b)(4)(i): Oxygen cylinders were stored near highly combustible material, especially oil and grease; or near reserve stocks of carbide and acetylene or other fuel-gas cylinders, or near other substance likely to cause or accelerate fire:    a)  CKJK, LLC dba Pellheat, South wall area of the Pellet Mill, on or about February 23rd, 2018 and time prior thereto:  Employees were potentially exposed to a fire/explosion hazard in that, six oxygen cylinders and four acetylene cylinders were stored together on the south wall area of the Pellet Mill building.

29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6:       a)  CKJK, LLC dba Pellheat, South wall area of the Pellet Mill, on or about February 23rd, 2018 and time prior thereto:  One acetylene cylinder was stored on the south wall area of the Pellet mill area without a valve protection cap affixed to the cylinder.      b)  CKJK, LLC dba Pellheat, South wall area of the Pellet Mill, on or about February 23rd, 2018 and time prior thereto:  One acetylene cylinder was stored on the south wall area of the Pellet mill area and was not properly secured to prevent the cylinder from tipping and/or being knocked over.  The chain used to the secure the compressed gas cylinders in the area was arranged in such a way that it did not tightly nest one acetylene cylinder and/or secure the acetylene cylinder which could allow the cylinder to be knocked over if impacted.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    a) CKJK, LLC dba Pellheat, Production areas, on or about February 23rd, 2018 and times prior thereto:  Employees voluntarily used disposable filtering facepiece respirators (dust masks) during cleaning operations of wood dust onsite.  Employees that used the dust masks were not provided with the information in Appendix D to this section.
