29 CFR 1910.119(j)(6)(ii): Appropriate checks and inspections were not performed to assure that equipment was installed properly and consistent with design specifications and the manufacturer's instructions. a. Zone D, C-203 CleanPac: On or about February 16, 2018, and at times prior, employees were exposed to hydrogen gas leaks and subsequent burns when the employer failed to ensure that the C-203 CleanPac, specifically the gasket installed on the CL500 flanged joint, was installed consistent with design specifications.
