29 CFR 1926.416(e)(1): Worn or frayed electric cords or cables were used: On or about February 26, 2018 and times prior thereto, at the worksite located across or near 1516 Rockwater Lane in North Little Rock, Arkansas, employees were using frayed electrical cords to supply power to hand tools such as, but not limited to, staple guns and hand saws. This condition exposed employees to shock hazards.

This condition exposed employees to fall for from heights hazards.

29 CFR 1926.503(c): The employer did not retrain affected employees who already had been trained but demonstrated inadequate understanding and skill required by paragraph (a) of this section: On or about February 26, 2018 and times prior thereto, at the worksite located across or near 1516 Rockwater Lane in North Little Rock, Arkansas, the employer had not retrained employees required to work above six feet or more on fall hazards and fall protection. Employees demonstrated an inadequate understanding of fall protection since they were working at heights greater than six feet without any type of fall protection. This condition exposed employees to fall for from heights hazards.
