29 CFR  1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a) Luis Gonzalez located at 6105 Alum Creek drive, Columbus, OH 43125:  On and preceding March 8, 2022, the employer did not ensure that roofers installing shingles using pneumatic nail guns wore appropriate eye protection thereby exposing them to struck-by hazards to the eyes.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  a) Luis Gonzalez located at 6105 Alum Creek drive, Columbus, OH 43125:  On and preceding March 8, 2022, the employer did not ensure that roofers installing shingles and other materials on a roof of a commercial building varying in height from 12 to 30 feet wore appropriate fall protection thereby exposing the employees to falls up to 30 feet.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) Luis Gonzalez located at 6105 Alum Creek drive, Columbus, OH 43125:  On and preceding March 8, 2022, the employer did not ensure that an extension ladder used for access to the roof of a commercial building had the side rails extending at least 3 feet beyond the upper landing nor was the ladders secured the top with a grasping device provided. The following condition exposed employees to falls up to 12 feet.
