29 CFR 1926.502(d)(6)(i): A snap hook not designed for this connection was engaged directly to webbing, rope or wire rope;   On or about 02/27/2018 at the 275 Ridge Road Construction Project, employees were wrapping the rope lifelines around roof joists then attaching the snap hook onto the rope lifeline.

29 CFR 1926.503(a)(2)(ii): The employer did not assure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:  On or about 02/27/2018 at the 275 Ridge Road Construction Project,  the employer did not have a competent person train the employees on the correct procedures to install and use the personal fall arrest systems in use.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.   On or about 02/27/2018 at the 275 Ridge Road Construction Project, employees were using an aluminum ladder to access the roof deck and no grasping device was in use.
