29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder: On or about February 28, 2018, at the above addressed site, employees were exposed to a fall hazard of approximately 12 feet when gaining access to and from a roof while using an extension ladder set-up in a location where it could be displaced that was not secured to prevent accidental displacement.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about February 28, 2018, at the above addressed site, employees were exposed to a fall hazard of approximately 12 feet when performing roof work on a low slope commercial roof while not being protected from falling to the lower level. Michael Kevin Walsh Roofing Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(11), which was contained in OSHA inspection number 1126314, citation number 1, item number 1 and was affirmed as a final order on 8/15/2016, with respect to a workplace located at 10218 SE Banyan Way in Tequesta, Florida 33469.
