On or about February 28, 2018, at the above addressed jobsite, employees working on the second floor, below carpentry operations, were not wearing a hard hat(s) to protect against falling materials and debris.

On or about February 28, 2018, at the above addressed jobsite, employees performing residential construction operations on the outside top plate, greater than twenty five (25) feet above the ground, were not protected from falling, thereby exposing employees to fall hazards.
