29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a) On February 18, 2026, at the above addressed jobsite, employees were exposed to eye injury while operating a pneumatic nailer and a circular saw without eye protection.     In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.   a) On February 18, 2026, at the above addressed jobsite, employees working from a carpenter's scaffold to set the top ridge, 13 feet above the interior of the second floor of the home, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls.  b) On February 18, 2026, at the above addressed jobsite, employees working from a carpenter's scaffold to set the top ridge, 24 feet above the interior of the first floor of the home, were not provided with guardrails, midrails, cross rails or other means of fall protection, thereby exposing the employees to falls.  No abatement certification or documentation is required for this item.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On February 18, 2026, at the above addressed jobsite, employees engaged in framing activities were exposed to falls of approximately 11 feet without the use of fall protection.  Employees were exposed to falls from the outer edge of the second story to the ground below.   No abatement certification or documentation is required for this item.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) On February 18, 2026, at the above addressed jobsite, the two extension ladder used by employees to access and egress to the second story of the home, measuring approximately 11 feet from the ground, did not extend to have the side rails extend at least three (3) feet above the upper landing.  No abatement certification or documentation is required for this item.
