29 CFR 1910.28(b)(11)(ii): Each flight of stairs having at least 3 treads and at least 4 risers was not equipped with stair rail systems and handrails in accordance with Table D-2, Stairway Handrail Requirements: On or about February 26, 2018, employees working at the Keyweld #4 machine were exposed to fall hazards while using the stairway to access the rear portion of machine and stairway did not have a stair rail system on each open side.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about February 26, 2018, employees working on the west side of the 5 foot Schlatter machine were exposed to pinch point hazards and the employer did not ensure that pneumatic cylinders mounted to the outside of the machine were guarded.
