29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. On February 22, 2018, in the Production Area, a mill operator was exposed to a pinch point hazard from the lack of point of operation guarding on the Farrel SN 44217, 14 inch mill machine.

29 CFR 1910.216(b)(1)(ii): Safety triprod. Installed in the front and in the back of each mill and located within 2 inches of a vertical plane tangent to the front and rear rolls. The top rods shall be not more than 72 inches above the level on which the operator stands. The triprods shall be accessible and shall operate readily whether the rods are pushed or pulled. On the Farrel, SN 44217, 14 inch mill machine the two push-pull hand brakes (round metal rod) located on each end of the face or the front and rear of the 14 inch mill roller was not located within 2 inches of a vertical plane tangent to the front and rear rolls.
