The unused portion of the band saw blade was not guarded.    B.)    TM Industrial Supply, Inc., 1432 Walnut Street, Erie, PA  16502, sheet metal fabrication area, on or about February 28, 2018 and times prior thereto:  Employee use a Birmingham H-6010 shear to preform work operations including, but not limited to, cutting sheet metal.  The hold downs on the Birmingham H-6010 shear were not guarded.  C.)    TM Industrial Supply, Inc., 1432 Walnut Street, Erie, PA  16502, machine shop, on or about February 28, 2018 and times prior thereto:  Employees utilize a Nardini ND 1585E lathe to perform work operations that include, but are not limited to, chuck turn shafts.  The lathe was not equipped with a chuck guard.
