29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.  (a)  On or about the week of February 19, 2018, the Diesel Caterpillar K (Model:  DP 50, S/N: 6CM __234) forklift had 8 feet extensions added to the forks exposing employees to struck by or tip over hazards.  The 8 feet extensions affect the capacity which could result in tip over, loss of steering control, falling loads, or collisions.  (b)  On or about March 1, 2018, the Caterpillar 50 J forklift had holes in the middle of each of the forks exposing employees to struck-by hazards.  By altering the forks without the manufacturer's assistance would compromise the strength and reducing the capacity of the forks.  (c)  On or about March 1, 2018, the Yale L (E11131) forklift had a hole on the driver's left side of the forks exposing employees to struck-by hazards.  By altering the forks without the manufacturer's assistance would compromise the strength and reducing the capacity of the forks.
