29 CFR 1910.151(b):  There was neither an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace nor a person or persons adequately trained to render first aid:    (a) KLX Energy Services LLC, at S Weld County Road 3E and E Weld County Road 16, McGirr Pad, Johnstown, CO: On 2/24/18, KLX Energy Services LLC did not ensure that there was either an infirmary, clinic, or hospital used for the treatment of all injured employees in near proximity to the workplace or a person or persons adequately trained to render first aid.  An employee sustained a finger tip amputation while handling steel pipe.  The injured employee was not provided first aid on site.  The KLX Energy Services LLC employee was driven by the well site's company man to UCHealth Medical Center of the Rockies, approximately 5.4 miles from the site for treatment.  The employer did not ensure that when a hospital was not in near proximity to a worksite, that a person is adequately trained to render first aid.
