29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  Harmony Hills Assisted Living, on or about February 21, 2018, workers performing roof sheathing work were not protected from falls of 20 feet or more the ground below.  Two workers were wearing harnesses, but both were not connected to a lifeline.  One worker was  observed carrying and nailing sheets of plywood without being tied off and the other was observed being tied off and then disconnecting the lifeline from his harness D ring.    Note:  Abatement certification and documentation are required for this item.     JD Framing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29CFR1926.501(b)(13), which was contained in OSHA inspection number 1105829, citation number 1, item number 1a and was affirmed as a final order on December 22, 2015, with respect to a workplace located at 1495 South Tech Lane, Meridian, Idaho 83642.

29 CFR 1926.503(a)(2)(iii): The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used.  (a)  Harmony Hills Assisted Living, on or about February 21, 2018, workers performing roof sheathing work were not protected from falls of 20 feet or more the ground below.
