29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used:  a. On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA employees were observed climbing the frame of the scaffold where there were no ladders in use for accessing and egress from the scaffold on the left side of the new construction home. Employees were exposed to fall hazards of up to 14-feet to the lower level.  b. On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA employees were observed climbing the frame of the scaffold where there were no ladders in use for accessing and egress from the scaffold on the front, right side of the new construction home. Employees were exposed to fall hazards of up to 7-feet to the lower level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section:  a. On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA employees were exposed to fall hazards of 14-feet to the lower level where guardrails were not installed at the working level of the scaffold on the left side of the new construction home.

29 CFR 1926.451(h)(2)(ii): A toe board was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below: Note: This requirement does not apply to float (ship) scaffolds where an edging of 3/4 inch x 1 1/2 inch (2 x 4 cm) wood or equivalent was used in lieu of toe boards.  a. On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA employees were working from a scaffold where  there were no toe boards installed on the left side of the new construction home. Employees on the ground were exposed to potential struck by hazards by materials including but not limited to bricks and hand tools.  b. On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA employees were working from a scaffold where  there were no toe boards installed on the front right side of the new construction home. Employees on the ground were exposed to potential struck by hazards by materials including but not limited to bricks and hand tools.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about 2/27/2018 at 3353 Laurel Leaf Way, lot 101, Buford, GA two employees were observed exposed to 10-foot fall hazards while performing bricklaying operations from the low slope roof where no fall protection was provided.
