29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):     (a)  MDZ Roofing, Ltd, at 10361 Cook Way, Thornton, CO 80229:  On and before December 6, 2017, the exposing employer did not ensure that each employee engaged in residential construction activities 6 feet or more above lower levels was protected by guardrail systems, safety net systems, or personal fall arrest systems.  Employees were performing roofing work on a two story condominium complex, with roof pitches up to approximately 6/12, without the use of fall protection.  This condition exposed employees to fall hazards of up to 20 feet.    MDZ Roofing, Ltd was previously cited for a repeat violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(13), which was contained in OSHA inspection number 1251979, citation number 1, item number 1 and was affirmed as a final order on 9/14/2017, with respect to a workplace located at 4689-4698 Flower St., Wheat Ridge, CO 80033.

29 CFR 1926.451(f)(5): Scaffolds were moved horizontally while employees were on them although the scaffolds were not designed for such movement by a registered professional engineer:    (a)  MDZ Roofing, LTD, at 10361 Cook Way, Thornton, CO 80229: On and before December 6, 2017, the exposing employer did not ensure that a scaffold was designed by a registered professional engineer when moved horizontally with employees on it.  The employer allowed employees to occupy an elevated platform that consisted of a trash bin with plywood on the sides and back, and with an open front, and with no guardrails in place on the front.  The platform was connected to a rough terrain forklift, and moved horizontally, with three employee riding on the platform.  No personal fall arrest systems were in use.  This platform was not designed by a registered professional engineer to support employees when moved horizontally.  This condition exposed employees to fall hazards and to the hazard of getting struck-by the forklift.
