29 CFR 1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was not protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system: (a) MDZ Roofing Ltd, at 2770 S Elmira Street, Denver, CO: On 1/17/2020, MDZ Roofing Ltd did not ensure that each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. Workers installed membranes upon flat roofs of a structure encompassing multiple units of a condominium. One worker operated a leaf blower to remove debris from the approximate 5:12 pitch roof over a garage. The worker performed this work while standing upon and moving about the pitched roof. Neither guardrails, safety net system, nor personal fall arrest system were in use. The worker was not protected from a fall. This condition exposed the worker to an approximate eight foot fall hazard. (b) MDZ Roofing Ltd, at 2770 S Elmira Street, Denver, CO: On 1/17/2020, MDZ Roofing Ltd did not ensure that each employee on a walking/working surface having an unprotected side or edge which was six feet (1.8m) or more above a lower level was protected from falling by the use of a guardrail system, a safety net system, or personal fall arrest system. Workers installed membranes upon flat roofs of a structure encompassing multiple units of a condominium. One worker performed work on the membrane at the edge of a flat roof while standing upon the approximate 5:12 pitch roof. The worker performed this work while standing upon and moving about the pitched roof. Neither guardrails, safety net system, nor personal fall arrest system were in use. The worker was not protected from a fall. This condition exposed the worker to an approximate eight foot fall hazard.

Workers installed membranes upon flat roofs of a structure encompassing multiple units of a condominium. Flat roofs were separated by chimneys and second floors of units, and were also separated by sections of pitched asphalt shingle roofs and were on different elevations. A single safety monitor was assigned to monitor the work of approximately five workers who worked on flat roof sections for units 39, 41, 43, and 45. The safety monitor was not on the same surface and within visual distance of all employees working on the roofs. This condition exposed the workers to an approximate eight foot fall hazard.

Workers installed membranes upon flat roofs of a structure encompassing multiple units of a condominium. Flat roofs were separated by chimneys and second floors of units, and were also separated by sections of pitched asphalt shingle roofs and were on different elevations. The foreman of the work crew was designated as the single safety monitor assigned to monitor the work of approximately five workers who worked on flat roof sections for units 39, 41, 43, and 45. The safety monitor had other responsibilities which took his attention from the monitoring function. This condition exposed the workers to an approximate eight foot fall hazard.

Workers installed membrane upon flat roofs using GAF EverGuard 1121 TPO Bonding Adhesive (containing toluene, acetone, n-hexane, naphtha, and cyclohexane) and GAF Everguard TPO Seam Cleaner (containing xylene and ethyl benzene). Workers were required to wear 3M Cool Flow tight fitting air purifying half face respirators when applying adhesive. MDZ Roofing Ltd had not established and maintained a written respiratory protection program. This condition potentially exposed the workers to a respiratory hazard.
