29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On the west side of the commercial building, employees engaged in roofing activities on a pitch roof of 2:12 without fall protection were exposed to a fall hazard of approximately 12 feet to the lower level.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous. On the south side of the construction site, employees using a Makita skill saw with the power cord missing the ground pin were exposed to electrical hazards.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws. On the south side of the construction site, employees using extension cords with the outer sheathing pulled away exposing the internal wiring were exposed to electrical shock hazards.
