29 CFR 1926.453(b)(2)(v): A body belt with a lanyard was not attached to the boom or basket when working from an aerial lift: On or about June 29, 2022, on the west side of the project site; an employee was utilizing an aerial lift without their personnel fall arresting equipment attached to the boom or basket at heights over six (6) feet. Alfredo Castillo was previously cited for a violation, or an equivalent, of the Occupational Safety and Health Standard 1926.453(b)(2)(v), which was contained in: a) OSHA Inspection #1452147 citation number 1, item number 2, with a citation issued January 23, 2020, and was affirmed as a final order on March 18, 2020, concerning a workplace located at 7710 N FM 620 Building 9, Austin, TX 78726
