Section (5)(a)(1): The employer failed to furnish to each of his employees employment and a place of employment which was free from recognized hazards that were causing or are likely to cause death or serious physical harm, in that employees were exposed to struck-by hazards:      (a) On or about February 11, 2018 for an employee who was injured after being struck by a 2 3/8 inch diameter tubular that fell out of a Keystone slip-type elevator that was configured to handle a 2 7/8 inch tubular at the Juneau 11-2H1 well site near Williston, North Dakota.    Abatement note: Among other feasible and useful means of abatement, the employer can develop policies and procedures designed to ensure the selection and use of appropriately configured elevators. For guidance, the employer can reference the procedures described in the operators manual for Keystone Y-Series elevators, the recommendations found in API RP 54, and the inspection criteria found in API Spec 8c.
