29 CFR 1910.176(a): Where mechanical handling equipment is used, sufficient safe clearances was not allowed for aisles, at loading docks, through doorways and wherever turns or passage must be made: a) Accuride Erie LP, 1015 East 12th St. Erie, PA, area between the Megaline and Line 5 Rosler. On or about March 9, 2018, an employee was injured when the mobile stair unit he was standing on was struck by a forklift maneuvering a turn. The forklift did not have sufficient clearance between the stair unit and pallets in the aisle to make the required turn.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: a) Accuride Erie LP, 1015 East 12th St. Erie, PA. On or about March 13, 2018, and at times prior there to, an in-patient hospitalization was not reported to OSHA within 24 hours. The accident and resulting in-patient hospitalization took place on or about March 9, 2018. The incident was not reported to OSHA until March 12, 2018.
