29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: a.) Accuride Erie, LP; 1015 E. 12th Street, Erie, PA - On or about 2/22/22, and at times prior thereto, contractors working in the company's facility had not been instructed on the facility's lockout procedures by the onsite employer (i.e. host employer). ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.333(b)(2)(ii)(A): Safe procedures for deenergizing circuits and equipment shall be determined before circuits or equipment are deenergized. a.) Accuride Erie, LP; 1015 E. 12th Street, Erie, PA - On or about 2/22/22, and at times prior thereto, a contractor's employee was cutting conduit to a live 480 volt panel box. The contracted employee had not been instructed in the electrical lockout and deenergization procedures by the onsite employer (i.e. host employer). ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
