Overlook, Cleveland, Ohio 44120, the employer did not conduct frequent and regular inspections of the job site where employees were exposed to fall hazards from working surfaces of varying heights approximately 14 to 21.8 feet.

29 CFR 1926.760(a)(1): Each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level was not protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems, or fall restraint systems. On or about March 16, 2018, during the installation of cross bracing on structural steel at the job site located at 11731 Mt. Overlook, Cleveland, Ohio 44120; the employer did not ensure working employees were protected from a fall hazard of 21.8 feet by utilizing a fall protection system.
