29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours: On January 31, 2018 at the worksite located at 283 Mountain Valley Water Place in Hot Springs, Arkansas, an employee received an injury resulting in an amputation. The employer failed to report the amputation to OSHA within twenty-four (24) hours as required.
