29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  Employees who performed servicing and/or maintenance task, such as but not limited to tool changes, on the three brake presses were not required to:   (1) know the type and magnitude of the energy, the hazards of the energy to be controlled, and the methods or means to control the energy,  (2) turn off and shut down the machines,  (3) physically locate and operate energy isolating devices in such a manner as to isolate the machine or equipment from the energy source,  (4) affix lockout or tagout devices to each energy isolating device,  (5) prior to starting work on machines that have been locked out or tagged out, verify that isolation and deenergization of the machine or equipment have been accomplished.   Employees requires to rely upon electronic control components were exposed to pinch point hazards from unexpectant release of hazardous energy on the Bystronic brake presses.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:   a) On or about March 14, 2018 the punch tooling on the Mubea Ironworker KLSH 550 was not guarded to prevent employee exposure to the point of operation hazard.   b) On or about March 14, 2018 the punch tooling on the Chicago 410-D press were not guarded to prevent employee exposure to the point of operation hazards.   c) On or about March 14, 2018 the ram on the hydraulic press in the Defense Department was not guarded to prevent employee exposure to the pinch point hazard created by the downward movement of the ram.   d) On or about March 14, 2018 the ram on the pneumatic press in the Defense Department was not guarded to prevent employee exposure to the pinch point hazard created by the downward movement of the ram.

Where the drive extends over other machine or working areas, protection against falling shall be provided. This subparagraph does not apply to manually operated sprockets:   On or about March 14, 2018 the sprocket and chain in the middle of the wall panel table in the Commercial Department was not guarded to prevent employee exposure to nip point hazards.
