A) Erich Starkweather DBA Starkweather & Sons Roofing & Siding on or about March 14, 2018, at the address of 3060 Airport HWY, Swanton, OH: The employer did not have a plan or program to identify job site hazards which assists employees in maintaining a safe working environment. Such programs would support the employer and employees in identifying and avoiding potential hazards, such as PPE, and falls during job site operations, decreasing the probability of employee injury and/or illness.

The employees using these tools in this condition were exposed to potential eye injuries from flying objects.
