29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:    Erich Starkweather dba Starkweather & Sons Roofing & Siding at the worksite located at 504 N. Defiance St. in Archbold, Ohio: On or about December 17, 2019, the employer did not ensure employees were protected from head injuries while working below employees performing roofing operations.  Employees tasked with clean-up operations were exposed to potential impact injuries from falling objects.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     Erich Starkweather dba Starkweather & Sons Roofing & Siding at the worksite located at 504 N. Defiance St. in Archbold, Ohio: On or about December 17, 2019, the employer did not ensure employees were protected from falls while performing tasks associated with roofing.  Employees performing roofing operations on a residential roof were exposed to falls of approximately 10 feet and up to approximately 22 feet from the unprotected edges of the roof.    Erich Starkweather was previously cited for a violation of this occupational safety and health standard 1926.501(b)(13) which was contained in OSHA inspection number 1301726, citation number 1, item number 4 and was affirmed as a final order on April 26, 2018, with respect to a workplace located at 3060 US Highway 20A, Swanton, Ohio, 43558.
