29 CFR 1910.178(m)(5)(iii): Powered industrial truck(s), with the operator dismounted and within 25 feet of the truck still in his view, did not have the load engaging means fully lowered.    On or about March 7, 2018, an employee was exposed to struck-by hazards after dismounting an industrial truck with the load engaging means (forks) elevated approximately three feet above the floor.  The dismounted employee was marking a load of two inch gang bolts located on the forks of her industrial truck when it was struck by another industrial truck causing the elevated load to shift and strike the dismounted employee.    Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
