29 CFR 1926.850(a): An engineering survey was not performed by a competent person to determine the conditions of the framing floors and walls and the possibility of unplanned collapse of any portion of the structure prior to permitting employees to start demolition operations. Panda Store Building, 2nd floor: Demolition work was ongoing and an engineering survey was not performed by a competent person. Employees were exposed to crush and struck-by hazards from accidental collapse.

29 CFR 1926.852(b): All materials chutes, or sections thereof, at an angle of more than 45 deg. from the horizontal, shall be entirely enclosed, except for openings equipped with closures at or about floor level for the insertion of materials. The openings shall not exceed 48 inches in height measured along the wall of the chute. At all stories below the top floor, such openings shall be kept closed when not in use. Front of Panda Store Building: A makeshift construction chute used to lower and remove demolition rubble to the ground was not entirely enclosed. Employees were exposed to struck-by hazards.
