On or about March 15, 2018, at the above addressed jobsite, employees performing roofing operations on a residential roof, approximately nine (9) feet four(4) inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.

On or about March 15, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
