(a) Cut and fold Operations of Weber Press #1017: On or about March 15, 2018, employees did not utilize established procedures to isolate energy sources to the die cutter machine, prior to clearing a jam of paper material from the die cutter. This exposed employees to pinch point/crushing hazards, and amputation injuries.
