29 CFR 1910.147(c)(4)(i): Procedures that were developed, and documented, were not utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.  (a) Cut and fold Operations of Weber Press #1017: On or about March 15, 2018, employees did not utilize established procedures to isolate energy sources to the die cutter machine, prior to clearing a jam of paper material from the die cutter. This exposed employees to pinch point/crushing hazards, and amputation injuries.

29 CFR 1910.147(c)(7)(i): The employer did not provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees:  (a) Cut and fold Operations of Weber Press #1017: On or about March 15, 2018, press operation employees attempting to clear a jam at the die cutter feed had not been trained to assess, understand and retain adequate knowledge and skills for safely clearing jams on the press. This exposed employees to pinch point/crushing hazards, and amputation injuries.

This exposed employees to multiple caught-in, crushing and in-going nip point hazards.    (b) Employees who worked on and around the die cut area of Press #1015 (Weber Model #A162 & Serial #1015), were not protected by guards affixed to the machine. This exposed employees to multiple caught-in, crushing and in-going nip point hazards.    (c) Employees who worked on and around the die cut area of Weber Press #1017 (Model #A224 & Serial #1017)   were not protected by guards affixed to the machine. This exposed employees to multiple caught-in, crushing and in-going nip point hazards.    The IFS Filing Systems LLC, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(2), which was contained in OSHA inspection number #1158856, citation number 1, item number 2, and was affirmed as a final order on July 13, 2017, with respect to a workplace located at 11225 W. Heather Avenue, Milwaukee, WI 53224.
