29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     The employer is failing to ensure employees are protected from fall hazards when conducting installation of shingles as part of re-roofing operations.  Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  The workers were exposed to falls of over 12 feet to the ground below.  This was most recently documented at 2108 S. 209th . Elkhorn, Nebraska on or about March 15, 2018 when the employer had employees working on the roof of this residence without any form of protective system.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.     The employer is failing to certify that employees have received fall protection training prior to allowing them to work on a residential style roof. Workers were observed installing shingles on a residential style construction site (house) without utilizing fall protection.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  The workers were exposed to falls of over 12 feet to the ground below.  This was most recently documented at 2108 S. 209th. Elkhorn, Nebraska on or about March 15, 2018 when the employer had employees working on the roof of this residence without any form of protective system and had not provided any training to them.

29 CFR 1926.1053(b)(1):  Were portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     The employer is failing to ensure employees are protected from fall hazards while utilizing portable ladders as part of re-roofing operations.  Workers were observed climbing up a portable extension ladder which did not meet the elements of this standard.  The ladder did not extend three feet above the upper surface being accessed by the employees on this site.  The workers were exposed to falls of over 12 feet to dirt and concrete below.  This was most recently documented at 2108 S. 209th. Elkhorn, Nebraska on or about March 15, 2018 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.

29 CFR 1926.1060(a):  The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards:      The employer is failing to provide employees with adequate and effective ladder safety training prior to allowing them to work on a residential style roof.  This training would allow each employee to recognize the hazards of falling and allow them to understand the proper procedures to be followed in order to minimize these hazards.  Workers were observed climbing up portable extension ladders which were set up incorrectly.  Had the employees had this training the following observed instances would not have been present on the job site.  The ladder did not stand three feet above the upper landing surface (roof) of the building.  The workers were exposed to falls of over 12 feet to dirt and concrete below.  This was most recently documented at 2108 S. 209th. Elkhorn, Nebraska on or about March 15, 2018 when the employer had employees working on the roof of this residence with the above listed issues present as they were going up and down the ladder on site.
