29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Old Angle Iron machine; On or about March 14, 2018 and at times prior thereto, employees required to change the punches on the old angle iron machine were exposed to the hazardous energies (pneumatic, hydraulic, and electric) present on the equipment, in that the employer failed to ensure that energy control procedures to control the hazardous energy sources on the equipment during punch changes were utilized.

29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a) Old Angle Iron machine; On or about March 14, 2018 and at times prior thereto, employees were exposed to amputation and crushing hazards while removing angle iron and flat iron from the machine in that the employer did not have guarding in place to prevent employees' hands/fingers from contact with the cutting blades.
