29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact. a. Personal protective equipment was not used as required. 1. Building 1 - On or about March 15, 2018 an employee was lifting and carrying metal roofing material by hand without wearing gloves to prevent lacerations. 2. Building 1 Roof - On or about March 15, 2018 employees were not wearing safety glasses while using nail guns. 3. Building 1 - On or about March 15, 2018 employees on the ground were exposed to struck-by hazards and head injuries by material being thrown from the roof.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. Building 1 - On or about March 15, 2018, roofers were exposed to a 27 foot fall while reroofing a building without fall protection.
