Employers shall select and require employees to use appropriate hand protection when employees' hands are exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes. a) Warehouse, LPG container storage: On June 19, 2017, and at times prior, the employer exposed employees to burn hazards while removing and replacing LPG containers on forklifts, in that proper skin protection was not provided.

29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a) Copper Wire Process Area: On or about March 5, 2018, and at times prior, the employer exposed employees to amputation hazards while cutting copper cables utilizing a hydraulic alligator shear (McIntyre Model 407 Serial number 36711996) that was not properly guarded.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) Warehouse: On or about May 04, 2017, and at times prior, the employer exposed employees to struck-by hazards by not taking Forklift (Serial Number P2924-0473-9862) out of service, in that the forklift had a frayed hydraulic hose that control the mast lift.
