29 CFR 1910.132(c): Personal protective equipment was not of safe design and construction for the work to be performed: a) On or about 3/16/18 - Exhaust Department, the welders assigned to in-line and off-line welding stations were exposed to contact with sparks and hot surfaces when welding. The short cuff of the knit glove used during welding, did not provide protection to the wrist and forearms. b) On or about 3/16/18 - Exhaust Department, the robot welder operators were exposed to contact with hot surfaces when removing parts. The short cuff of the Kevlar knit glove did not provide protection to the wrist and forearms.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: a) On or about 3/16/18 - Facility, Exhaust Department, the employer had not trained employees on the hazards associated with exposures to hexavalent chromium. The training should include potential exposures during work processes and the engineering and work practices that should be used to control exposures.
