29 CFR 1910.147(c)(7)(i)(A): Each authorized employee shall receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. On or about March 12, 2018, the employer did not ensure press operators authorized to perform service and maintenance activities, such as die changes (setup), on the Bliss C-75 mechanical power press, known as press EP10 Connecting Bar Press, received training in the recognition of hazardous energy sources, the type and magnitude of the energy available, and the methods and means necessary for energy isolation and control of the press.

29 CFR 1910.217(b)(7)(ix): If foot control is provided, the selection method between hand and foot control shall be separate from the stroking selector and shall be designed so that the selection may be supervised by the employer. On or about March 12, 2018, the employer did not ensure the selection between foot control operation and two hand control operation of a Bliss C-75 mechanical power press, known as the EP10 connecting bar press, was supervised. The key was left in the press control panel selector switch and used by press operators without employer supervision.

29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10. On or about March 12, 2018, the employer did not insure fixed barrier type point of operation guards were properly applied and adjusted per 1910.217 Table O-10 to prevent the hands and/or fingers of the operator of a Bliss C-75 mechanical power press, known as the EP10 Connecting Bar Press, from entering into the point of operation.
