29 CFR 1910.147(d)(4)(iii): Tagout devices utilized were not affixed in such a manner as to clearly indicate that the operation or movement of the energy isolating devices from the safe or off position was prohibited: (a) Dow Bayport Facility: On March 15, 2018 and at times prior to, the employer did not ensure that the tagout devices on the suction and discharge valves of pump P173 for the sulfuric system were affixed in such a manner as to clearly indicate that the operation or movement of the valves from the closed position was prohibited while performing a pressurized leak test. This condition exposed employees to chemical burns.
