29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics was not provided to the operator when the operator was involved in an accident or near-miss incident: On or about February 22, 2018 an employee was operating a forklift moving material from one building to another and while entering the building struck the industrial overhead garage door with the forklift resulting in the garage door to come off the track. The company failed to give the employee refresher training after the accident to ensure the operator has the knowledge and skills needed to safely operate a forklift. Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).
