29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) 11th Street, Rockford, Illinois - On or about March 22, 2018, the employer failed to ensure that a safe means of egress was provided to an employee working in an excavation in excess of eight (8) feet deep. An A-frame ladder that did not extend to the top or bottom of the trench was in use. There is no abatement certification or documentation required for this item.

The employee was exposed to caught-in hazards from falling soil /concrete. There is no abatement certification or documentation required for this item.
