29 CFR 1910.28(b)(3)(iii): Each employee was not protected from falling into a stairway floor hole by a fixed guardrail system on all exposed sides, except at the stairway entrance:   Facility: On 03/23/2018, a stairway, allowing access to a lower level of the building, was open-sided on three sides.  Employees using adjacent storage racks would be exposed to falls in excess of nine (9) feet onto the stairway and the lower level of the building.
