29 CFR 1910.95(d)(1): When information indicates that any employee's exposure may equal or exceed an 8-hour time-weighted average of 85 decibels, the employer shall develop and implement a monitoring program.    On or about April 5, 2018, the employer had not developed and implemented a monitoring program to determine employee's exposure to noise.  The following employees working within the DeKalb Materials Department were exposed to elevated noise levels that exceeded the 8-hr TWA (time weighted-average) of 85 decibels:    a) A saw operator was exposed to an 8-hour TWA noise level of 88.2 dBA, with a dose of 78.0%, during a 394 minute sampling period.  Exposure calculations included a zero increment for the 86 minutes not sampled.    b) A magnaflux #2 operator was exposed to an 8-hour TWA noise level of 90.8 dBA, with a dose of 112.1%, during a 402 minute sampling period.  Exposure calculations included a zero increment for the 78 minutes not sampled.    c) A magnaflux #4 operator was exposed to an 8-hour TWA noise level of 89.9 dBA, with a dose of 98.4%, during a 408 minute sampling period.  Exposure calculations included a zero increment for the 72 minutes not sampled.    No abatement certification or documentation required for this item.

29 CFR 1910.95(l)(1): The employer did not make available to affected employees or their representatives copies of 29 CFR 1910.95 and did not post a copy in the workplace.  On or about April 5, 2018, the employer did not make 29 CFR 1910.95 available to affected employees and did not post the standard in the workplace when the following employees working inside the DeKalb Materials Department were exposed to elevated noise levels that exceeded the 8-hr TWA (time weighted-average) of 85 decibels:  a) A saw operator was exposed to an 8-hour TWA noise level of 88.2 dBA, with a dose of 78.0%, during a 394 minute sampling period. 29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about March 23, 2018, magnaflux operators working inside the DeKalb Materials Department were required to grind on metal parts. The employer did not provide adequate guarding on portable hand grinders or protective shielding to protect others walking by from flying sparks.  Abatement certification is required for this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.1026(d)(1): The employer did not determine the 8-hour time-weighted average (TWA) exposure for each employee exposed to chromium (VI) in accordance with either paragraph (d)(2) or paragraph (d)(3) of this section.   On or about March 23, 2018, magnaflux operators working inside the DeKalb Materials Department was were required to perform grinding operations on stainless steel parts and the employer did not perform initial air monitoring to determine the 8-hour time-weighted average exposure for each employee exposed to chromium (VI).   There is no abatement certification or documentation required for this item.
